About the Rosenfeld Medical Home Visit Program
The UCLA Division of Geriatrics is proud to extend its commitment to exceptional senior care with our Home-Based Primary Care Program, serving residents of West Los Angeles and Santa Monica. This innovative program is designed to meet the needs of homebound older adults who generally live with multiple ailments, such as heart disease, arthritis, diabetes, and dementia.
Home visits allow medical professionals to identify problems promptly by recognizing early stages of an illness, enabling them to stabilize a patient instead of hospitalizing them. Regular home visits help older adults age more comfortably by bringing much-needed medical expertise to them and can prevent or delay transition to a nursing home and limit the need for post-hospitalization rehabilitation.

Through personalized, high-quality care delivered in the comfort of home, we ensure patients receive the attention and support they need while maintaining their dignity and independence. Our team of dedicated healthcare professionals work to create individualized care plans that address the unique needs of each patient, with additional support from a social worker and access to a telemedicine pharmacist also made available.
How our care works:
One of our geriatricians, along with a nurse practitioner, provides in-home care to older adults, regardless of income level, who are unable to regularly visit their doctors.
- Comprehensive New Patient Visit (60 minutes): Performed by a geriatrician, this initial visit includes a complete evaluation and assessment of past medical history, current condition, in-home support, social situation, and discussion of long-term healthcare goals with the patient (and family, if applicable).
- Follow-up Visit (20-60 minutes): Typically performed every 4-8 weeks by one of our geriatricians or a nurse practitioner, these visits help manage chronic conditions and assess any new needs.
Eligibility Criteria
- UCLA patients age 65 and older
- Homebound
- Resident of West Los Angeles or Santa Monica
