UCLA geriatricians provide comprehensive medical care for our patients in skilled nursing facilities or nursing homes. Whether patients need short-term rehabilitation after a recent hospital stay or require long-term care, we offer services at several community-based nursing homes in West Los Angeles and the Northwest Valley area.

Our goal is to provide continuity of care throughout the transitional period,  whether as an outpatient, or a resident in a nursing home.

When to Consider a Nursing Home?

Nursing home care can be a vital option for patients and their families following an acute hospitalization, particularly when patients are not yet medically stable enough to return home or to a community setting. For short-term needs, skilled nursing facilities (SNFs) offer rehabilitation and nursing care designed to support recovery, helping individuals regain function or until they reach a new baseline or plateau. In addition to rehabilitation services (physical, occupational and speech therapy), SNFs provide a range of skilled care including intravenous (IV) antibiotics, gastrostomy (G-tube) management, and wound care. Care is delivered primarily by nursing staff, while physicians conduct periodic in-person visits and offer 24/7 on-call availability as needed.

Long-term nursing or custodial care provides round-the-clock medical support for individuals who can no longer care for themselves. Nursing homes can also ease the burden on family members who may lack the resources to provide care at home. Additionally, nursing homes can offer palliative and hospice care services for patients requiring more comfort-focused support; hospice enrollment may require clarification on insurance coverage.

What to Look for When Choosing a Nursing Home?
When selecting a nursing home, patients, their family members, or medical advocates should visit the facility and request a guided tour. It’s important to note that nursing facilities typically have different staffing ratios compared to hospitals and acute care settings. While 24/7 nursing supervision is provided, one-on-one care is not standard and may require supplemental private caregiving to meet specific needs.

We encourage you to speak with the admissions director to address your questions and clarify expectations, ensuring alignment with your goals for a successful transition. You may also request to meet the Director of Nursing and the Nursing Home Administrator for additional insights. Interactions with these key staff members can provide a clearer understanding of the facility’s commitment to care and its overall environment.
