Before You Arrive
Check and confirm your essentials
A smooth arrival starts before you leave home. Please complete these steps:
- Opt in for appointment reminders: Skip the phone calls and get your appointment reminders by text. It’s quick, easy and convenient. Learn how to sign up.
- Confirm your appointment details: Double-check the date, time and exact location of your clinic or hospital visit.
- Save time with PreCheck-in: If you received an email or text message for pre-registration through myUCLAhealth, complete it beforehand to save time on the day of your visit.
- Referrals and authorizations: Confirm with your primary care provider or insurance that all necessary referrals and pre-authorizations are in place.
- Let us know you’ve arrived for an appointment: You can use your smartphone to tell us when you've arrived at your appointment. The MyChart app sends a message to your phone 15 minutes before your appointment asking if you’ve arrived. As you enter the clinic, tap the button on your phone to let us know you’ve arrived. Follow these steps.
What to Bring
Your day-of-visit toolkit
Regardless of whether you’re visiting for an outpatient appointment or a hospital stay, please bring the following items:
- Photo ID: Driver's license, passport or state-issued identification
- Insurance card(s): Current health insurance card and, if applicable, your Medicare or Medi-Cal card
- Copay/payment: To pay any required copay or deductibles
- Current medication list: Including dosages and frequency for all prescription and over-the-counter medications
- Patient forms: Any paperwork or forms sent to you by your provider that you haven't completed online
