At the HEaL Trauma Program, we understand that healing doesn’t end when a patient leaves the hospital—and that true recovery involves more than physical care. Our trauma-informed programs are designed to address the social, emotional, and psychological effects of trauma, ensuring every patient has the support they need to heal and thrive.

Our trauma recovery services

Community-based social care

Our Community Health Workers (CHWs) help patients navigate the complex challenges that impact recovery—such as housing, employment, and mental health—through personalized, ongoing support following hospital discharge.

Mental health support 

In partnership with UCLA Psychology, our program offers trauma-informed therapy, mental health assessments, and support for both patients and providers—all embedded within the trauma care experience.

Post-discharge PTSD screening

Our automated outreach system proactively screens patients 30 days after discharge for signs of PTSD, followed by personalized follow-up and direct access to care if needed—because timely support can change the course of recovery.
