Jose Bojorquez, CHW

Community Health Worker

Jose Bojorquez, CHW, serves as a Community Health Worker with the HEaL Trauma Program. He supports patients in the community after hospital discharge, helping them navigate resources and maintain continuity of care. Jose works closely with inpatient trauma social workers to identify each patient’s unique needs and provide personalized support.

Theresa Tobias, AA, CHW, known affectionately as TT, plays a vital role in the operations and growth of the HEaL Trauma Program. She is dedicated to empowering survivors as they reintegrate into the community, helping address social drivers of health (SDOH), reduce the risk of re-traumatization, and improve overall quality of life.

Kate Clair, MSN, RN, CEN, TCRN, MICN is a highly experienced Trauma Program Manager at UCLA Health. With over 15 years in emergency and trauma care, Kate is passionate about advancing trauma systems, mentoring nurses, and improving patient outcomes.

​Erik Coll, MPH, BSN, RN, serves as the HEaL Trauma Program Manager and Trauma Injury & Violence Prevention Coordinator. With over two decades of experience in emergency medicine, he focuses on injury prevention and holistic trauma recovery programs.

Daniela Salinas, MPH, is the Research Manager for the HEaL Trauma Program and an Injury Prevention Specialist at UCLA. She holds an MPH in Applied Epidemiology and a BS in Public Health. Her expertise includes protocol development, IRB submissions, data analysis, and publication support.
