Affiliate Rotation Guidelines
Before completing any other site steps:
- Annual TB Test & E2
- Make an appointment at UCLA Occupational Health to get TB tested and for E2 form completion.
- Bring the appropriate form with you for completion (see below).
- Send your program coordinator a copy of the completed form for your file.
- Annual Flu Shot
- Annual Declination Form (2024-2025)
- FIT Testing
- Must be done annually in order to be properly processed at affiliates sites. Reach out to your Program Coordinator if you are unable to attend any of the sessions below.
- Fit Testing Website
- BLS/PALS certification
- Submit a request for approval to avoid paying a fee. Notify the coordinator you're signing up so they can approve the course. Make sure you have your UID, a contact number and to use your mednet email.

- Go to the LA County of Department of Health Services Information Sheet and complete the information. Select "Harbor" and "Maryellen Alvarez". Once complete, Maryellen and Human Resource will approve your rotation.
- If you are new to county, please choose NEW from the electronic application found on the link above.

New Residents
- A county Livescan is only required once for all New residents. Please complete this two weeks prior to your rotation.
- Livescan hours 8 a.m. – 4 p.m. No appointment is needed. Make sure to bring a valid driver’s license or passport. Human Resource contact number is 424-306-4900
- Please email Maryellen and request Orchid Training one month in advance of your rotation

New Residents
- Please Note: There is a new onboarding system (APDS) the VA uses to process new incoming residents which includes TMS/Fingerprinting/Background Checks/Swearing-In/Badging/etc. Please look out for initial emails from the VA/Dwight John that will be followed by MVI. System emails. You will also receive emails from VA Personnel Security regarding fingerprint/background check submission information.
Returning Residents

- Contact VA Coordinator if you cannot access your TMS account as it will need to be re-validated and enabled.
- VA PIV Badge
- Please check the expiration date on your badge.
- You are cleared for patient care if your badge is current.
- If badge has expired or will expire soon, contact VA Coordinator to schedule livescan appointment at least 2 months in advance of your rotation.
- Contact VA Coordinator to make sure you are sponsored before scheduling badge renewal appointment. You will be provided with the most current badge appointment scheduling information.
Fingerprints are required for badges that are expired.

CHLA transitioned in May 2024 to an online application system to onboard affiliate rotators intending to start their rotations in the upcoming academic year. MedHub will serve as the platform for clearing all rotators. Prior to the start of a rotation, applicants will receive an electronic application link to fulfill our mandatory clearance requirements. These changes aim to streamline both the clearance and approval processes.
Key points to note:
- Paper packets will no longer be accepted.
- An updated clearance packet is required each academic year; historical data will be stored in MedHub for electronic updates.
- The approval process requires a minimum of 4 weeks from the submission of all necessary documents/information.
- Rotators are responsible for completing the packet in a timely manner to ensure adequate processing time.
- Annual updates for TB and flu vaccines are mandatory.
