Why Can’t You Just Remove the Thyroid Nodule?
This is a very common question we get from patients with thyroid nodules:
Can’t you just take out the nodule and leave the rest of the thyroid?
The short answer is no, and there are three key reasons why:
1. The Nodule Is Embedded Within the Thyroid
Thyroid nodules do not usually grow like grapes hanging off of the thyroid gland that can just be plucked off. They grow from within the thyroid tissue itself and are often deeply embedded within the substance of the thyroid gland. Because of this, removing just the nodule while leaving the surrounding thyroid intact is technically not feasible in most cases.
2. We Need Margins to Diagnose Cancer
There’s always a non-zero chance that the nodule could be cancer. To determine whether it is, pathologists need to examine the margin—the edge where the nodule meets the surrounding normal thyroid tissue.
Cancer is often identified by its ability to invade nearby tissue. So if we only remove the nodule without any margin, we can’t properly evaluate whether it’s benign or malignant.
3. Reoperation Increases Risk
Let’s say we remove just the nodule and leave some thyroid behind. If the final pathology later shows cancer, we may have to go back and do a second surgery to remove the rest of the thyroid on that side.
Unfortunately, reoperations are more difficult and risky because of scar tissue, which obscures delicate structures like nerves and parathyroid glands.
To protect you from that risk, it’s safer to remove the whole lobe (or entire thyroid) the first time, so we never have to go back into scarred tissue on that side.
