Participate in UCLA ATLAS Community Health Initiative
How do I participate in the UCLA ATLAS Community Health Initiative?
Participation in the UCLA ATLAS Community Health Initiative is safe, fast, and easy.
- CHECK YOUR APPOINTMENT REMINDER - If you have registered to receive text message reminders for your upcoming appointments at UCLA, the next appointment reminder you receive by text may include a link to guide you through the consent process. You will only receive the link if our records indicate that you have not already completed the Universal Consent for biological samples.
- If you have an in-person appointment at any of our participating locations, upon your arrival you will be presented with an iPad and asked to watch a short video which will go over the goal of the UCLA ATLAS Community Health Initiative and how you can be a part of it. After reviewing the material, you will be asked if you would like to opt-in or opt-out.
How do I withdraw from the UCLA ATLAS Community Health Initiative?
Your participation in the UCLA ATLAS Community Health Initiative is voluntary. If you agree to participate but change your mind, we will honor your request. To ensure you have the time to think over your decision carefully, there will be a 10-day window from the time you sign the consent to the time your sample becomes available for researchers. Even after the 10-day window, you can change your mind.
To stop the use of your sample or data in a study, you can contact the UCLA ATLAS Precision Health Biobank at 310-825-4136.
Any unused samples will be removed from the UCLA ATLAS Precision Health Biobank and any researcher who received your samples will be asked to destroy them. Please note that we cannot retrieve materials that have already been used by researchers.
