Effective Communication
- SBAR Communication Tool 
- The SBAR communication tool is to improve communication between the nursing student, clinical instructor, and primary RN to ensure patient safety and promote Zero Harm. The tool can be used before and after medication administration, performing tasks, and procedures.

If you forget your School ID Badge while on-site:
- Visit the Security Desk on the first floor of the facility 
- Check in with your Name, Affiliation: Nursing Student, School Name, and photo ID to obtain a visitor pass

Student Injury at UCLA Health Facility
- Escalation and Notification 
- Follow School Handbook policies for notifying faculty and/or program lead
 - On your unit/department: notify the primary RN/preceptor and charge nurse
 
 - Blood Exposures Only: students will follow the same steps as an employee laid out on the following forms: 
- Occupational Exposure Requisition - Employee Form 
- Follow the instructions on Page 2
 
 - Occupational Health Requisition - Source Patient 
- Follow the instructions on Page 2
 
 - Incident Report - Complete prior to going to the Occupational Health office (during business hours) or the Emergency Department (outside of business hours). 
- Note: The Workers’ Compensation does not apply to students.
