      Provider Resources
The UCLA Alzheimer's and Dementia Care Program provides a variety of services for established patients and caregivers, which include the following:
- Caregiver education regarding disease progression, management of behavioral disturbances
 - Caregiver Support
 - Advance Care Planning
 - Referrals to neurology, psychiatry, or geriatrics
 - Referrals to community-based organizations
 - 24/7 access
 
Patient Eligibility
For your patient to be eligible for the UCLA Alzheimer's and Dementia Care Program, the patient must meet the following criteria:
- Must be referred by a UCLA Primary Care Physician, Neurologist, or Psychiatrist who is willing to collaborate with the Dementia Care Specialist
 - Have a confirmed diagnosis of Alzheimer's or other types of dementia
 - Have a caregiver(s), preferably a family member, who is willing to participate in care
 - Not reside in a skilled nursing facility or be enrolled in hospice care
 
How to Enroll Your Patient
If you are a UCLA Primary Care Physician, Neurologist, or Psychiatrist and you would like to enroll your patient in the program, please place a referral on Care Connect to the ADC Program.
