• Most recent colonoscopy with pathology report
• Most recent imaging reports (CT scans/ MRI reports)
• Colonoscopy and pathology reports during diagnosis
• Any recent clinical notes and laboratory values
• Insurance information
• Name of specialty pharmacy
• If previously on biologic medication, dose and frequency of the medication given
• List of previous medications used and dates when used
• List of previous surgeries and dates of the surgeries
