What is the clinic?

Approximately one-third of patients with inflammatory bowel disease are diagnosed in childhood or early adolescence.  As these individuals enter adulthood, a smooth transition out of the pediatric setting is especially important — both to ensure effective continuity of care as they begin to see new providers, and to empower them to take ownership of their ongoing healthcare needs.  The UCLA IBD Pediatric to Adult Transition Clinic for 18-26-year-olds ensures our young adults are able to navigate this evolution successfully.

What makes our clinic unique?

As a pediatric gastroenterologist who also sees young-adult patients, Dr. Hassan Hamandi will help to make the transition easier by overseeing the clinic while patients begin to interact with adult ancillary staff and experience other components of an adult health care setting. The transition clinic - one of only a few in the country - includes a focus on issues specific to this population, such as sexual health, counseling on alcohol and drugs, vaccinations, and vocational and academic support.  The transition clinic is also supported by a multi-disciplinary team that includes nurse practitioners, registered nurses, dieticians, psychologists and social workers.  There is also a strong collaboration with adult gastroenterology providers and colorectal surgeons, working in conjunction with IBD subspeciality programs in nutrition and women's health. 

Goals of the UCLA IBD Pediatric-to-Adult Transition Clinic

  • Empower young adult IBD patients to exercise graduated medical autonomy in managing their disease while still under the care of pediatric providers, although in an adult clinical setting. 
  • Foster a safe space to discuss topics pertinent to young adult IBD patients, including sexual health, counseling on alcohol and drugs, vaccinations, and vocational and academic support. 
  • Support young adult IBD patients through a comprehensive center of excellence with a multidisciplinary team. 
  • Transition care to adult gastroenterology providers who are integrated into the medical practice.
