Obtaining Your Medical Records

We offer several ways for you to obtain your medical records from UCLA West Valley Medical Center.

Option 1: Request your medical records from the patient portal.

 

myUCLAHealth Patient Portal

You can request your medical records by following these steps:

  1. Log in to myUCLAhealth on your personal computer. Medical records cannot be requested via mobile device at this time.
  2. Follow instructions using the below links:

    2. Download Your Requested Medical Records

If you would like to create a myUCLAhealth account, you can view instructions on how to sign up.

MyHealthONE Patient Portal

If you received care at West Hills Hospital or UCLA West Valley Medical Center before March 11, 2025, you can access your medical records in MyHealthONE. You can sign in online or call 855-422-6625 for portal support. 


Option 2: Follow this link to complete the medical record authorization form electronically.

Have your photo identification ready and follow the prompts. Once completed, the form will be sent to our medical records team directly.

Option 3: Download and print the authorization for Release of Health Information form.

To submit completed medical records authorization forms by email, please send to: [email protected]

To submit completed medical records authorizations forms by fax, please fax to 310-983-1468. 

To submit completed medical records authorizations forms by mail, please mail to: 10833 Le Conte Ave., CHS BH-902 Los Angeles, CA 90095.

Payment: There is a small fee to obtain your health information. Please make check/money order payable to UC Regents 

  • To obtain radiology images, please contact the radiology department directly at 818-676-4745.
  • To obtain a certified copy of your birth certificate please contact your state or local vital statistics office.
  • Records delivered by mail will be shipped within 5 to 7 business days after processing.
  • Records delivered by email will be received within 1 to 2 business days after processing.
