The UCLA Health Adult Cancer Survivorship Consultative Clinic, also known as the VITA (Vital Information and Tailored Assessment) Cancer Survivorship Clinic, is a specialty consult for patients with a prior history of cancer.  VITA can provide focus on the after-effects of cancer treatment and guidance in the type of follow-up that patients may need.  Patients can benefit from a referral to ensure that cancer specific follow-ups are in their care plan, promotion of disease prevention, and emphasizing one’s health and wellness such as weight control and regular exercise.

Who Qualifies?

Patients with a prior history of cancer, who are one to two years past their acute cancer treatment. These patients may still be experiencing some residual symptoms or at risk for long term complications and need preventative health screening and education on the topic.

Who Can refer?
