Preparing for your visit with a mesothelioma specialist

We want to make the most of your visit with us. To help our mesothelioma experts understand your diagnosis and treatment history, please gather and send us the following medical records before your appointment. If you need help at any step, don’t hesitate to reach out. We're here for you.

What to send us

Please collect and submit the items below. These help our team review your case thoroughly before your appointment:

1. Radiology reports and images

• All imaging reports (CT scans, PET-CTs, MRIs, etc.)
• Image files should be burned onto CDs
  Then, upload them to UCLA using our secure link.
  ➤ If you’re unable to upload, you may FedEx the CDs to us instead (address below).

2. Pathology report and slides

• Final pathology report from your biopsy
• Contact the Pathology Department where your biopsy was done
  ➤ Ask them to send your pathology slides (1 tissue block preferred) directly to our office (address below).

3. Operative or procedure notes

• Any notes from procedures related to your biopsy

4. Doctor's notes

• Oncologist consultation note and/or your most recent history & physical

5. Treatment records

• Flowsheets for chemotherapy and/or immunotherapy (if you’ve received either)

6. ID and insurance

• A clear copy of your driver’s license
• The front and back of all health insurance cards
